this sounds a lot like a kidney infection or maybe even an infected tooth. best to get him to the vet right away.
stop giving him cow's milk too. older cats especially have problems digesting it.
how long has he not been eating?
have you tried different kinds of food? our cats don't eat anything with fish, so it could just be that he's picky and won't eat what you're feeding him.
what kind of climate do you live in?
is he drinking water?
is he having any digestive problems? if an animal is runny, that's usually a sign of either something going through its system or sickness.
